Written by MyIslandJob from the job title and employer. See the employer's page for the full description.

The Community Support Worker will support patients receiving care at home through the Hospital@Home service, helping them recover safely outside hospital. Duties may include personal care, mobility support, monitoring patients, recording observations, assisting with daily activities, and reporting changes to registered nurses or other clinicians. Candidates should have experience in health or social care, good communication skills, compassion, reliability, and the ability to work independently while following care plans and safeguarding procedures. A willingness to undertake training, work flexible shifts, travel between patients’ homes, and collaborate with the wider multidisciplinary team is expected. Relevant care qualifications and experience in a community or hospital setting are advantageous.

About the employer

Wye Valley NHS Trust provides hospital and community healthcare services across Herefordshire and surrounding areas, including Hereford County Hospital, community hospitals, outpatient services, and care delivered in patients’ homes. The Trust works with local health and social care partners to provide safe, effective, and compassionate treatment close to where people live. Its Hospital@Home service supports suitable patients in their own homes while maintaining hospital-level clinical oversight, helping improve patient experience and reduce unnecessary admissions or length of stay. Employees generally work within multidisciplinary teams and receive access to NHS benefits, occupational support, learning opportunities, and pension arrangements. The Trust serves a largely rural population, so community-based staff need flexibility, sound organisation, and the ability to travel between locations.

Working in United Kingdom

The United Kingdom offers opportunities across healthcare, technology, engineering, education, finance, retail, and skilled trades. NHS and social care employers regularly recruit internationally, although vacancies may require professional registration, recognised qualifications, or specific experience. British culture is diverse and generally values punctuality, privacy, fairness, and clear communication. Lifestyle varies from busy cities to quieter rural communities; public transport is strongest in urban areas, while rural workers may need a driving licence and car. Living costs are highest in London and parts of southern England, while Wales, northern England, Scotland, and Northern Ireland can be more affordable. Most overseas workers need a visa, commonly the Skilled Worker visa, with an eligible sponsor and minimum salary requirements; some health roles qualify for the Health and Care Worker visa. Check current government rules, registration requirements, housing costs, and NHS recruitment guidance before relocating.
