Job Description

This role supports the District Nursing Team Leader in managing and coordinating community nursing services across Powys. The successful candidate is expected to be a registered nurse with relevant community or district nursing experience, strong clinical assessment skills, and the ability to supervise staff and coordinate caseloads. A community nursing qualification, or willingness to work toward one, may be required under Annex 21 development arrangements. The post involves visiting patients in their homes, supporting complex care, promoting independence, maintaining accurate records, and working closely with GPs, hospitals, social care and other professionals. Candidates should demonstrate leadership, sound clinical judgement, flexibility, excellent communication, and a commitment to safe, compassionate, person-centred care.
