Job Description

The Population Health Management Lead will lead the use of population data, health intelligence and partnership working to identify health inequalities, understand local needs and improve outcomes across communities. Requirements typically include substantial experience in population health, public health, NHS or healthcare planning; strong analytical and data interpretation skills; and the ability to translate evidence into practical programmes. Experience working with integrated care systems, primary care, local authorities and community organisations is desirable. The successful candidate should be confident managing projects, influencing senior stakeholders, presenting complex information clearly and supporting service improvement. A relevant degree or equivalent professional experience, along with knowledge of NHS policy, health inequalities and population health management approaches, is generally expected.
