Job Description

The PCN Clinical Pharmacist will provide expert medicines-related care across South Fulham primary care practices. Responsibilities typically include structured medication reviews, supporting patients with long-term conditions, improving prescribing safety and quality, managing medicines optimisation projects, responding to medication queries, and working closely with GPs, nurses and community services.
